What is refractive lens exchange?

Refractive lens exchange (RLE) is a permanent vision correction procedure in which your natural lens is replaced with a custom-calculated artificial intraocular lens (IOL). It uses the same surgical technique as cataract surgery, but is performed before a cataract has developed, with the goal of reducing or eliminating dependence on glasses and contact lenses.

RLE is particularly suited to patients over the age of 55, though it can be considered in younger patients who are beginning to experience presbyopia (the age-related loss of near vision) or who have high prescriptions that are not well suited to laser vision correction. Because the natural lens is removed, there is no possibility of developing cataracts in the future.

At Vale Eye Surgeons, RLE is performed by a consultant ophthalmologist with an MSc in cataract and refractive surgery and extensive experience in premium lens implantation and refractive outcomes.

How does it work?

Through a tiny self-sealing incision at the edge of the cornea, the natural lens is gently removed using phacoemulsification. A premium intraocular lens is then inserted in its place. We offer a range of lens options including monofocal, toric, extended depth of focus (EDOF), and multifocal designs, allowing your surgeon to tailor your vision to your lifestyle.

What to expect

1. Consultation

A comprehensive assessment including biometry, corneal tomography, and macular OCT to confirm your suitability and calculate the ideal lens power. Your surgeon will discuss lens options in detail, explaining the visual trade-offs of each choice.

2. Surgery

The procedure is performed under local anaesthetic (eye drops) as a day case, taking 15-20 minutes per eye. We typically perform same-day bilateral surgery - treating both eyes during the same visit - to provide a more balanced and efficient recovery. Most patients describe the experience as completely painless.

3. Recovery

Most patients notice dramatically improved vision within 24-48 hours. You will use eye drops for several weeks and attend follow-up appointments to check healing. Final visual results, particularly with multifocal lenses, continue to improve over three months as the brain adapts to the new lens.

Is RLE right for me?

RLE is best suited to patients over 55, but can be considered for younger patients with presbyopia, high hyperopia, or high myopia unsuitable for laser correction. Your surgeon will give you an honest assessment of whether RLE, laser vision correction, or a phakic lens implant would give you the best outcome based on your eyes and lifestyle.
